Tooth discoloration after direct pulp capping using calcium-silicate containing material

I. Introduction

When pulp is exposed due to trauma or dental caries, direct pulp capping is one of useful treatment options. In direct pulp capping, calcium silicate-containing materials such as MTA or Theracal LC® are used. However it is known that these Calcium silicate-containing materials can cause discoloration on teeth. This case reports discoloration on teeth after direct pulp capping using calcium-silicate containing materials.

II. Case Presentation

1. Chief complaint : I think there is caries in the tooth that had undergone dental treatment.
2. Sex/age : M/24
3. Past Medical/Dental History : N.S / Direct resin filling with direct pulp capping using MTA(2 years ago)
4. Present Illness : Darkish discoloration, per(-), mob(-), ice spray(+), EPT(+) on #16
5. Impression : Tooth discoloration on #16
6. Tx Plan : Removal of discolored dentin and resin inlay.

1. Chief complaint : My tooth that had undergone dental treatment turned black
2. Sex/age : F/22
3. Past Medical/Dental History : N.S / Reattachment and direct pulp capping using Theracal LC® (3 years ago)
4. Present Illness : Darkish discoloration, per(-), mob(-), ice spray(+), EPT(+) on #11
5. Impression : Tooth discoloration on #11
6. Tx Plan : Removal of discolored dentin and direct resin filling.

III. Discussion and conclusion

Direct pulp capping using calcium silicate-containing materials on vital teeth is a useful treatment option to preserve teeth vitality. However, many studies reported that calcium silicate-containing material can cause tooth discoloration. Though the reason and mechanism of tooth discoloration are not fully understood, but several factors such as contamination with blood, contact with sodium hypochlorite, presence of light and oxygen can contribute to tooth discoloration. Clinicians should keep in mind that the tooth discoloration may occur when calcium silicate-containing material is used for direct pulp capping and make an effort to avoid discoloration on tooth.
